Jolie adopted the Vietnamese infant Pax Thien in March, but because she and Pitt are unwed it was tricky for them to adopt him as a couple under Vietnamese law and Jolie signed as his parent, giving him the name Pax Thien Jolie.
Jolie asked a court in Santa Monica to change his name to Pax Thien Jolie-Pitt. That would mean he shares the same surname as their three other children - Maddox, Zahara and Shiloh.
The filing was first reported on the celebrity website TMZ.com on Tuesday.
